By attending classes, events, services, activities, and using the studio facilities and equipment provided by Focus Boutique Fitness Limited (the “Company”), you hereby acknowledge that there are certain inherent risks and dangers in the strenuous nature of classes.

You acknowledge that you have been fully informed of the strenuous nature of this exercise program and the possibility of adverse physiological occurrences including, but not limited to abnormal blood pressure, fainting, heart attack or death. You acknowledge that you have voluntarily chosen to participate in an intense physical exercise.

You confirm that you are in good health and suffer from no disease or congenital disease which would prevent yourself from utilization of services and you hereby declare that you do not conceal any information regarding congenital disease or health from the Company which would result in refusal of my membership or participation in any of their classes or use of their services.

You acknowledge and agree to always exercise responsibly and with due care and attention to your own medical and mental condition.

You understand and accept all risks of injury from studio activities and using any studio equipment. You understand and agree that the studio, and any other studio operated by the Company that you visit, do not, to the extent permitted by law, accept any liability for any injury, disability, death, or loss or damage to person or property, except those arising from their negligence or breach of this Agreement.

You assume all risk for your health and well-being, and agree to assume full responsibility for you, and anyone you booked for with this account, and fully release, indemnify, and hold harmless the Company and its entities.

You agree that to the maximum extent permitted by law, the Company or its entities will not be liable to you under any theory of liability. Without limiting the foregoing, you agree that, to the maximum extent permitted by law, the Company or its entities will not be liable for any indirect, incidental, consequential, special, or exemplary damages, loss of profits, business interruption, reputational harm, or loss of data (even if foreseeable) arising out of or in any way connected with your use of, or inability to use, the services.

We make no promises with respect to, and expressly disclaim all liability for: (1) products, services, information, programming, and/or anything else provided by a third party that is accessible to you through the services; or (2) the quality or conduct of any third party you encounter in connection with your use of the services.

Your sole remedy for dissatisfaction with the services is to cease use of the services.

I acknowledge that the Company does not provide medical or nursing services including medical diagnosis for any physical conditions. Further, I acknowledge that exercising and usage of all equipment, including lockers provided by the Company may result in risk or danger. Therefore, I agree to accept all the risks and agree that the Company, by their shareholders, authorized directors, representatives, and employees shall not be liable for my injury, illness, death or damages of my property which may directly or indirectly arise from utilization of services at the Studio.

General Safety

The Company reserves its right to ask any member who does not comply with the orders, rules, and regulations to leave the Studio.

Trainer / Instructor has the authority to stop a member from participating in class if it is considered that it may cause injury, and the member and guest of member must comply with the trainer’s instruction. 

The Company advises members not to bring any valuables into the Studio because the Company will not be responsible for any loss, damage, or theft of personal property of the member or guest of the member.

Model Release

The member hereby assigns and grants the Company and its legal representatives the irrevocable and unrestricted right to use and publish the photographs and videography.

You hereby acknowledge that you are of Legal Age (18 years) and you agree to comply with the Terms and Conditions set forth.
